I have answered all the questions in this form honestly and I am aware if I have agreed to have any of the listed medical conditions, I will need to consult my GP before commencing an exercise programme and get medical clearance. If I am affected by any of the listed conditions at a later date, I also agree to inform my personal trainer on any changes in health and fitness. I agree and understand that taking part in an exercise programme will be done at my own risk and that the trainers and PardaFit will not be held liable for any injury, condition or any other ailment associated with the attendance of the programme.
